The case history presents the influence of physical and particularly emotional stress in diabetic and hypertensive female patient who took care of her disabled husband for a long time. Her husband – suffering from diabetes as well – lost his self-sufficiency following leg amputation and became totally dependent on the care of his wife. As a reaction to the new life conditions he changed his behavior to his relatives and particularly to his wife. The strenuous care for immobile husband resulted in patient’s long-term stress that evidently caused decompensation of her diabetes and worsening of her global health
status.

In the presented case history we describe a patient without health-care insurance who recently experienced diabetes mellitus and did not seek medical care. He was transported to the hospital in advanced diabetic coma. We present serious decompensation of diabetes mellitus with extreme laboratory parameters levels and complications that were successfully managed by enhanced effort of physicians and intensive
care.

Meglitinides, such as repaglinide and nateglinide, are recently introduced short-acting agents. They stimulate insulin secretion by binding near the sulphonylurea
receptor/ATP-sensitive K+ channel, and they are used to control postprandial hyperglycaemia. They can also be used in conjunction with metformin or insulin (bed time insulin
therapy).

The authors present a case study of an obese female patient with diabetes inadequately controlled by diet and oral antidiabetic drugs. Intensive education during hospitalization resulted in the improved control of diabetes and the weight reduction. The authors want to demonstrate that patient’s motivation to comply with treatment can be improved in many cases only after the patient perceives immediate danger of late diabetic complications during
hospitalization.

In the presented case history we demonstrate female patient with type 2 diabetes mellitus, obesity accompanied by hypertension and impaired lipid metabolism. Following acute coronary event and professional intervention together with an introduction of “The new daily stereotype of diabetic patient” patient changed her attitude to her disease. Even though it was extremely time consuming, the combination of adequate medication and appropriate education proved to provide long-lasting effective results and considerable improvement of patient’s quality of life. It is evident that sufficient motivation of patient can help us to reduce substantially the expensive
medication.

This paper describes a case of the post-partum autoimmune thyroiditis, with transient hyperthyroidism and subsequent shift into hypothyroidism. Symptoms of the disease were so discreet that remained unnoticed both by the patient and her neighbors. The substitution treatment was based on the laboratory examination, as the function of patient’s thyroidal gland had been followed up due to the abnormal screening results in the 11*th* week of pregnancy (slightly increased TSH and thyroid peroxidase
antibodies).
