The hypoglycemic syndrome is a complex of predominantly
vegetative and neuroglycopenic symptoms that are present in
hypoglycemia. It is mostly caused by hypoglycemizing medication but it
can occur as a physiological reaction or as a symptom of a grave
illness.
In three case histories we documented the occurrence of hypoglycemic
syndromes of similar symptoms that were caused by different conditions.
There are differences in the progress of hypoglycemic symptoms that may
help to determine the most probable etiology. Both taking a medical
history focused in hypoglycemic events and measuring glycemia in the
time of present symptoms represent the basic diagnostic procedures that
should enable any physician to choose subsequent procedures.

The paper describes a case of a female patient
with uncontrolled diabetes mellitus and acute Charcot
osteoarthropathy who achieved a metabolic compensation only
after an introduction of an intensified insulin treatment using
insulin analogues. A reduction of diabetes instability and
improved levels of glycated hemoglobin significantly contributed
to a positive effect on the course of osteoarthropathy.

The case of a treatment of ulcers at dorsum and heel of
a left lower extremity in a female patient with underlying diagnosis of
diabetes mellitus type 1 is presented. The condition was complicated by
lymphedema and a newly diagnosed osteomyelitis. A systemic antibiotic
treatment, local ulcer treatment by modern healing remedies combined
with a complex treatment of lymphedema led to a gradual healing of the
ulcers during the hospitalization.

A case of 79 years old female with the syndrome of
primary hyperparathyreosis and a bulky cystically transformed adenoma of
a parathyroid gland in defunct thymus in the anterior mediastinum is
documented. 6×5.5×5cm sized tumor did not exhibit malignant signs.
As a result of extended physiological variability of a parathyroid gland
formation during ontogenesis, the finding of an atypically located
adenoma of parathyroid gland in a cervical or thoracic area is
relatively typical. Almost one quarter of all adenomas then develop
outside the usual localization of parathyroid glands. An accurate
preoperative diagnosis of ectopic adenoma may prevent a possible failure
of a surgery.
