This case describes 27-year-old woman with anamnesis of suicidal attempt. She took alcohol, flunitrazepam, triazolam and self-administered long reacting insulin Semilente MC (40 IU/ml) cca 200 IU
s. c. As a result of insulin intoxication and prolonged hypoglycemia there was severe damage of mnestic and intellect abilities and a personality
degradation.

Atypical adult-onset type 1 diabetes (1b) represents new nosological entity. It is a rare
disease, representing only about 10 % of type 1 diabetes. It is characterized by clinical signs
of type 1 diabetes, however the islet cell antibodies are lacking as well as the predisposing
HLA haplotypes. We report on two cases of this atypical form of type 1
diabetes.

Secondary diabetes mellitus originated on the base of
pancreatic tissue destruction. Acute pancreatitis belongs to infrequent
causes of secondary diabetes. Origin and further development was dependent
on the extent of the damage to pancreatic tissue and possibility of its
regeneration. Clinical case of secondary diabetes originated in patient
with acute necrotic pancreatitis was presented.

The paper deals with the problems of the occurrence of ulcus cruris in elderly people suffering from diabetes. He stresses the significance of a comprehensive evaluation of geriatric patients embracing inter-disciplinary cooperation to identify further possible risks. Taking into consideration polymorbidity as well as measures of tertiary prevention, the development of illnesses and the age of patients require a plan for long-term treatment, assuring regular check ups by specialists and nursing in a home
environment.

Case study of 55-year-old patient after tail of pancreas resection, with complete gastrectomy and esophagojejunoanastomosis in 1984 due to tail of pancreas nesidioblastoma. The patient had been rehospitalised after 5-year asymptomatic period on the Orthopaedo-traumatologic clinic due to right arm tumour with subsequent pathological fracture of humerus. In the histological finding from humerus nesidioblastoma metastasis was confirmed. Owing repeated hypoglycaemia the patient was mowed to Internal clinic. By progression of the illness authors describe further treatment of the
patient.

This casework describes a 41-year-old patient, whom we hospitalized at our Intensive Care Unit with severe bacterial heart failure (NYHA IV). This patient had congenital combine aortal deficiency with prevailing stenosis and suspicion to infective endocarditis. His state was complicated with acute renal impairment of pre-renal etiology with low minute volume and anasarka. We managed to stabilize the patient after 5 weeks with the use of continuous renal replacement therapy, we arranged the internal environment and prepared him for a cardio surgery, which he successfully passed and return to a common
life.

This case describes hospitalization of 27-year-old woman, ex-nurse, who has undergone thyroid gland surgery several times and also treatment with radio-iodine. Her state has been repeatedly described as relapsing thyrotoxicosis. We have detected and confirmed a factitia thyrotoxicosis when she was hospitalized at our
clinic.

This case describes development of autoimmune thyroiditis with transient hyperthyreosis and subsequent transition into hypo function in leuproreline (gonadoliberine analog) treatment of 83 old man with prostate carcinoma. Specialized publication describes so far rare cases of autoimmune thyroiditis development during long-term treatment with gonadoliberine analogs (GnRH analogs). Fact, that gonadoliberine significantly inflicts the immune system regulation, indicates that described caseworks may be in causal relationship with GnRH analog application and it does not have to be just an incidental capture. Even though there is still missing unique evidence to prove this (studies evaluating incidence of newly developed autoimmune thyreopatitis in treatment with GnRH analogs), a thyreoidologic anamnesis and TSH screening examination is in place for all patients treated long-term with GnRH
analogs.
