We describe a case of a 41-year-old man with diabetes mellitus type II, who underwent one unsuccessful operation and two cuccessful leg saving arterial reconstruction. Majority of calf muscles necrosed due to prolonged ischemia. The necrosed tissues were cut surgically throught fasciotomias. Maggots of Lucilia Serricata were used for wound clean up.

Unusual therapeutic approach in newly diagnosed diabetes mellitus. Without creativity and individual approach to the patient is impossible to achieve the objectives (blood glucose as close to normal as possible and normal life) outlined in the DCCT and in the St. Vincent Declaration in everyday clinical practice. After a random high glucometer blood glucose reading on Friday afternoon and its laboratory confirmation (21,5 mmol/l) instead of sending the shocked newly diagnosed diabetic (Ms. I. P., age 29 years, employee of the Therapeutic Spa Bardejovské Kúpele) to hospital I decide to start immediately with intensified insulin therapy. Following a short education I. P. applies the first doses of Actrapid HM insulin with Novopen (Novo Nordisk) herself. The newly diagnosed patient refuses to be on the sick - list and on Saturday enters service in the spa. Four doses of short acting insulin are ordered according to her blood glucose values by phone. By Sunday, on the same regime the blood glucose values do not exceed 12 mmol/l. On Monday complete laboratory investigation is performed and the therapeutic regime is modified to a basal - bolus system. Ms. I. P. is now near euglycemic and fully compensated both physically and psychically. Despite the confirmed diagnosis of IDDM she enjoys her work and life and she does not feel that great changes occurred in her life.
